Tirzepatide (Mounjaro)is a once-weekly dual agonist of the glucose-dependent insulinotropic polypeptide (GIP, also known as gastric inhibitory polypeptide) receptor and the glucagon-like peptide-1 (GLP-1) receptor, developed by Eli Lilly.

Tirzepatide is a peptide drug composed of 39 amino acids, with certain amino acids structurally modified. In May 2022, it was approved by the U.S. FDA as a first-in-class medication under the brand name Mounjaro. By mimicking the body's natural incretin hormones, it helps patients with type 2 diabetes achieve strong glycemic control and significant weight loss.

It activates GIP and GLP-1 receptors in the gut, helping you feel full faster, stay full longer, and use blood glucose more efficiently. It helps to:

In addition, in April 2023, Eli Lilly initiated a head-to-head trial of tirzepatide versus semaglutide for the obesity indication (SURMOUNT-5) to compare their efficacy and safety over 72 weeks, with semaglutide dosed at 2.4 mg.

Weight loss usually begins within 2–4 weeks after starting treatment, and appetite suppression can be noticed within just a few days. Most patients lose about 5–8% of their body weight by week 12, with continued weight loss over the following 6–12 months. The maximum weight loss effect is typically seen around 72 weeks of treatment.

If you miss a dose, take it within 4 days of your scheduled dosing date. If more than 4 days have passed, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one. Occasionally missing one or two doses usually does not significantly affect overall treatment outcomes.

Most patients experience some weight regain after discontinuation, with about 50–70% of the lost weight returning within 12 months. Maintaining lifestyle changes, including dietary adjustments and regular exercise, can slow this rebound. Some patients remain on a lower maintenance dose long term to help sustain weight loss.

Combination therapy requires careful medical supervision. Tirzepatide can be safely combined with metformin and may enhance its effects. Combining it with other GLP-1 receptor agonists does not provide additional benefit and increases the risk of side effects. For stimulant-based weight loss medications, risks and benefits should be evaluated on an individual basis.
